If you were trying to record underwater, then it might be leaking.  Check for any visible damage on the case and the o-ring.  If you were not underwater, then moisture may gotten into the case when you were putting the camera inside and fogged up the case.  Before placing the camera in the case, make sure you are in a dry area and that no water is in the case.
