Update: if anyone has this problem; in my case through process of elimination I was able to narrow it down to the motherboard. Did a quick skim and didn’t see any obvious problems with the motherboard, so I just replaced it to save the customer time and money since these things are from 2005 and not worth much, and I already had a replacement motherboard in stock.
