Can you "scan" a document OK as opposed to "copy" a document?
Does it happen when using the scanner flatbed glass, the ADF or both?
Have you tried to reset the printer back to its' default settings and check? Go to Setup, and press the OK button.
Press the up or down arrow button, select Restore Default Settings.
Press the OK button.
