You must make sure that the headset is plugged into the controller BEFORE you turn the Xbox console on.

Same problem, headset works sometimes other times it doesn't. The problem seems to be with Microsoft's software and how it detects the headset when plugged in. Update (07/24/2020): You must make sure that the headset is plugged into the controller before you turn the Xbox One console on with the controller.
