Could the Echo be pairing via Bluetooth to a device that isn't audible? I've found the Echo's Bluetooth to be annoyingly persistent.

I have had that problem too. Sometimes when I ask it something (like what's today's weather) it will flash like it is answering me, but says nothing. Later, it will work fine. Disconnecting and reconnecting the power does seem to fix it, but it will do it again a few days later.

Mine started doing that too. It still makes a tone to let me know it is listening, but doesn't respond verbally, play music, or make any other sounds. Turning it off and back on fixes it temporarily, but it starts doing it again a few days later.
I have 2 Echo Dots. One is 2nd Gen, the other is 3rd Gen. I am only having this problem with the 3rd Gen.
