@diyarecet could be an issue with the charging port. Make sure that there is no debris in it and that the pins are not bend or corroded. Of course, also check your charger and your charging cable. If all of that is ok, you would need to disassemble your phone and check the charging port (luckily on your phone there is a charging board which is easily replaced). Change the battery as well. Don't forget to check the flex cable from the charging board to the main board. Those do come loose and may have a corroded connector etc.

Since iFixit does not yet have a guide for this, use something like this video Just skip the part about the port replacement. Change the whole board.

While you work on your phone, take lots of pictures. Once you are done with it, take those pictures and create a guide for iFixit. That will help the next person that has the same problem as you do.
