My PS4 fan is out of control

My PS4 fan works great but after I play or start to do something starts to speed up and before I know it it sounds like it's about to take off and spins so fast I have to turn it off before it explodes or something I've done everything I know I have took it completely apart to clean it rear the system to factory reset it I think the fan is good because it does great until it starts to work I guess so when it starts to do this my system starts to lag and go slow I don't know what I need to fix but some help would be greatly appreciated thanks for your time
